Issue with registration page

I have been contacted by a returning member attempting to register on the boards. It seems that at the end of the registration form is a "security question", but there is only a box with no question. If someone could take a look and get back, I would greatly appreciate it.

Be sure they keep the spaces in the registration code, a few players leave them out:(
